Comprehensive Family Medicine Patient's Responsibility

Patient care requires a team effort now more than ever with the field of medicine becoming more complex. The more you know and research about your health the better informed you will be in decision making and your care will be more complete. We ask that you take the responsibility to familiarize yourself or write down the following information to have with you at your appointment.

Personal Health History to include:

  • Current medical problems
  • Past medical history and hospitalizations
  • Past surgical history including cosmetic procedures
  • Invasive testing
  • Immunization history
  • Current medications / prescriptions
  • All over the counter medications, supplements, vitamins, herbals, and topicals
  • Allergy history to medications or foods
  • Preventive health screening dates, last physical, pap smear, mammogram, prostate screening, colon screening

Family History
Knowing family history of health and illnesses, diseases inherited or not, and cause of deaths.

Being On Time for Appointments
Arriving late disrupts work flow and causes those patients after you to wait as well. Please arrive on time, 20 minutes before your scheduled appointment if you are a new patient.

Prompt Notification of Cancellation
Please notify the office immediately if you are unable to make your appointment which will allow us the time to give that appointment to a patient in need.

Ask Questions
Please feel free to ask any member of the staff questions at anytime, especially Dr. McHenry. Use Dr. McHenry to filter the information you gather outside of the office. Be informed about your health issues.
